我上個月開始定期洗腎，可以就近請洗腎診所開立失能診斷書申請失能給付嗎？
被保險人如因罹患傷病，經治療後，症狀固定，需終身定期做透析治療(洗腎)，須洽「經衛生福利部醫院評鑑優等以上、醫院評鑑合格之醫學中心或區域醫院、醫院評鑑及教學醫院評鑑合格」之全民健康保險特約醫院專科醫師診斷出具失能診斷書，再由醫院逕寄勞保局，才符合失能給付申請規定。
貼心提醒
因洗腎申請胸腹部臟器機能失能給付，應由符合上述層級的醫院出具失能診斷書，但澎湖縣、金門縣、連江縣等離島的被保險人可由原就診的全民健康保險特約醫院或診所診斷出具，不受前述規定的限制。

If the insured person’s status is stabilized but must regularly undergo kidney dialysis treatment after suffering from injury or disease and receiving treatment, a diagnosis of disability.
Should be issued by national health insurance special contracted hospital, which is rated as
excellence or better in Hospital Accreditation, or as qualified in Hospital Accreditation (for
medical center and regional hospital), or as qualified in Hospital Accreditation and Teaching Hospital Accreditation by the Ministry of Health and Welfare.
The hospital must submit the diagnosis to the Bureau of Labor Insurance to meet the requirements of disability benefits.
Reminder
Those applying for disability benefits due to dysfunctional thoracic or abdominal organs require a disability diagnosis from an accredited hospital as described above; however, insured persons residing in offshore islands such as Penghu County, Kinmen County, and Lianjiang County are exempt from these restrictions and may submit a diagnosis from a NHI hospital or clinic from their original place of treatment.
